Position Summary:

The Registered Nurse (RN), in conjunction with the consumer's medical providers, leads the team in the education, delivery, administration, including observed self-administration, monitoring and documentation of medication. The RN will support consumers in transitioning into, and maintaining, their tenure in the community. The RN will begin engaging consumers while they are still residing in state psychiatric hospitals and work with them on developing a WRAP. The RN will assist consumers in developing self-care skills to help them manager their unique medical needs. The RN will participate in team meetings, assist with medical case management, and provide training and education to all program staff. CSS staff. Care Managers will perform the duties and responsibilities listed below provided they are authorized to perform the duty or responsibility under their professional licensure, certification, or credentialing standards.

Duties and Responsibilities:
  • Contributing to the development, implementation, monitoring, and updating of the individualized rehabilitation plan pursuant to NJAC 10:37B-2.4
  • Performing therapeutic rehabilitative skill development pursuant to NJAC 10:37B-4.2
  • Offering services appropriate to meet the consumer's needs pursuant to NJAC 10:37B-4.6
  • Performing crisis intervention pursuant to NJAC 10:37B-4.4(a)24
  • Coordinating and managing services as directed.
  • Administering and monitoring prescribed medications and injections
  • Keeping accurate records of all medications
  • Adhering to policy and procedure regarding medication administration and storage
  • Providing education regarding medications and health related issues
  • Coordinating continuity of care with medical services in the community
  • Participating, with other team members and consumers/family/others as appropriate, in the completion and updating of treatment plans for each consumer within established time frames or as indicated by consumer need.
  • Completing progress notes accurately, addressing treatment goals, major treatment issues and changes in mental status.
  • Assisting with case management and intra/interagency coordination of services for consumers.
  • Responding to consumer defined crises 24 hrs per day, 365 days per year, providing crisis intervention and consultation to consumers, their families and others
  • Monitoring medication intake by assessing symptoms, side effects and medication compliance, reporting to team.
  • Maintaining a NJ driver's license.
  • Engaging in supportive counseling and provides education in daily living skills and self-care skills.
  • Engaging with program participants in a warm, empowering manner.
  • Exhibiting a person-first approach in all interactions and interventions.
  • Providing skills-building education in daily living skills.
  • Assisting individuals in developing Wellness and Recovery Action Plans and Psychiatric Advance Directives.
  • Initiating linkage with community services.
  • Providing interventions to stabilize the community living situation.
  • Performing outreach, engagement, and assessment to enroll consumers into the program
  • Providing 24/7 on-call support for consumers.





Licensure/Certification:

Current and valid license to practice as Registered Nurse

Current and valid NJ driver's license



Minimal Qualifications:

Education: Graduate of accredited nursing school

Experience: At least one year experience working with consumers diagnosed with a severe and persistent mental illness.

Blood borne pathogen training required.





Equipment Use: Computer, copier, facsimile
