POSITION SUMMARY

The LIFE Connection Navigator acts as a liaison between program participants (including their caregivers) and PACE departments ensuring improved patient flow and overall care management. Interacts with all levels of department staff to support daily operations of care delivery in a way that supports customer satisfaction. Supports the coordination of participant scheduling within and between Interdisciplinary Team Members to optimize service encounters and overall implementation of the participant Plan of Care. Demonstrates knowledge of all PACE services, programs, providers and resources. Provides orientation of new PACE participants and caregivers to the PACE model of care in the form of individual outreach and group education.

The LIFE Connection Navigator will assist the participant in development of service requests to the interdisciplinary team and communicate between parties as appropriate. Coordinates the service request authorization process in conjunction the appropriate members of the interdisciplinary team and LIFE connection finance departments.

Assists the interdisciplinary and management team in resolution of participant grievances. Works with social services to complete appropriate applications for services in conjunction with LIFE Connection program participants. Support the activities programming in the LIFE Connection center by working with the Activities Coordinator to develop and conduct participant activities. Coordinates ordering and delivery of durable medical equipment.

The LIFE Connection Navigator provides exceptional customer service to all contacts.. The LIFE Connection Navigator assesses participants' needs accurately through conversation. The navigator gathers and documents complete and accurate client information using skills designed to elicit information for an appropriate and efficient response to requests. Demonstrates strong problem solving skills to appropriately respond to complex situations and protects client confidentiality at all times.

This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to team work and cooperation.

QUALIFICATIONS

EDUCATION: Associates degree required. Bachelor's degree preferred.

LICENSE/CERTIFICATION: Valid NJ Driver's License, Registration and Insurance are required.

EXPERIENCE: Minimum 3-5 year's health care knowledge required. Also, minimum one year experience, working with the elderly required. Experience in nursing or social work preferred. Proficiency in Clinical Applications preferred at time of hire; incumbents within position will be trained appropriately and then skill will be required for this position within 30-60 days from date of hire.

PERFORMANCE EXPECTATIONS

Demonstrates the competencies as established on the Assessment and Evaluation Tool for this position.

WORK ENVIRONMENT

This positon must be able to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the PACE participants' care. The work setting is in an Adult Day Health Center and primary care clinic environment with moderate noise levels and controlled temperatures.

This position also requires corrected vision and hearing to normal range. Incumbents must be able to communicate with all staff, vendors, and clients. Requires manual and finger dexterity and eye-hand coordination; the ability to use department equipment. This position also requires lifting/carrying up to 65lbs.and pushing or pulling 150lbs.using appropriate body mechanics and equipment, with reasonable accommodation if needed.

This positions reports to department leadership and supervises incumbents as assigned.
