As a member of the T1D Exchange Quality Improvement and Population Health Team, you will join an innovative team of experts using big data and quality improvement methodology to advance diabetes care. In addition to impacting the lives of people living with diabetes, you will benefit from the national impact of your work, mentorship, and numerous publication opportunities.

Job Responsibilities include:

  • Lead and support clinical quality improvement projects and initiatives, which may include collaborating with providers and other care team members to improve workflows that impact quality performance:
    1. Coach and train national teams on quality improvement (QI) tools and methods.
    2. Predict and identify barriers to project implementation, develop strategies to address them, and support completion.
    3. Establish strong working relationships with the teams to improve performance.
    4. Facilitate bi-monthly QI meetings, and committee meetings with clinical teams
    5. Draft project reports, case studies, and implementation summaries
    6. Create and update QI performance dashboards.
    7. Analyze data and develop run/control charts to demonstrate changes.
    8. Develop QI and Population Health content for national teams
    9. Other duties as assigned.
  • Population Health Data Analysis/Health Equity
  1. Implement aims, policies, and processes that are focused on improving health equity and reducing overall health disparities
  2. Collaborate with clinical teams and drive health equity initiatives
  3. Analyze data and develop run/control charts to demonstrate trends in equity
  4. Train clinical teams on Racism and Health Equity
  5. Assist in the preparation of manuscripts and abstracts for presentation at scientific conferences
  • Serve as the Project Manager on Special Projects including industry-funded initiatives. Key responsibilities include:
    1. Define and own a special project roadmap and vision
    2. Provide staff support for meetings, committees, special projects, and conferences.
    3. Manage execution of assigned projects
    4. Act as the primary liaison between external QI stakeholders and internal teams
    5. Develop project reports and publications
    6. Evaluate and prioritize issues and enhancement requests
  • Manages project timelines, agenda development, minutes and documents, slide decks, and preparation of reports on project status and activities for various audiences
  • Demonstrates the ability to write concise, informative reports and QI change packages

Basic Qualifications:

  • Bachelors Degree required; Masters degree in health care related field strongly preferred with 2-5 years of full-time Quality Improvement experience
  • IHI Basic certificate in Quality/CPHQ/ Lean Six sigma Green Belt preferred
  • Highly motivated and mission-driven, with a strong commitment to health equity
  • Utilizes appropriate data analytic techniques to analyze and display data as evidenced by completed graphs and charts
  • Self-motivated and demonstrate excellent judgment in problem-solving when developing daily work priorities
  • Ability to work both independently and within a team environment and a multi-dimensional environment
  • Must have effective communication skills, relate positively with the community, and communicate with utmost tact and diplomacy
  • Previous experience coaching QI teams and facilitating QI training
  • Previous experience leading Health Equity intiatives
  • Proficiency in Microsoft Office suite and project management tools/ software
  • Above all, the candidate must continuously improve and contribute to the team's overall objectives
