POSITION SUMMARY

The Care Management Lead Care Manager provides leadership to the interdisciplinary team responsible for care management, care coordination and discharge planning. The Lead Care Manager ensures patients move effectively through the continuum of care in an effort to provide appropriate, safe, quality care in a cost-effective manner. The Lead Care Manager acts as a resource for staff and offers input/assists with complex cases, discharge dispositions and other daily issues that occur. Duties include: assistance to the Director of Care Management through oversight and support of staff, case review, scheduling, monitoring length of stay and resource management and Emergency Department Case Management activity when requested. The Lead also provides care management to a defined population of patients, and continued mentorship of the care management team. Ambulatory Lead Care manager will assist Director in oversight of CM caseloads, care plan& assessment completion, potential case identification, and other HealtheCare related tasks.



KNOWLEDGE, SKILLS& ABILITIES
  • Experience in Care Management/Care Coordination
  • Knowledge of, Medicare and Medicaid (AHCCCS) regulations required
  • Knowledge of AtlantiCare's Value Based Programs/Contracts
  • Excellent written and verbal communication skills.
  • Establish relationships to influence decisions, gain cooperation, and facilitate change with Providers, all levels of management, employees and supervisors of other Hospital, Ambulatory and community partners.



PERFORMANCE EXPECTATIONS


    • Oversees the day to day operations of the team in absence of management.
    • Provide care management at the unit/practice level in a reduced capacity.
    • Manage daily campus staffing including call outs.
    • Participate in the organizational Daily Safety Huddle.
    • Review all ACO admissions for appropriateness.
    • Participate in weekly ACO Huddles.
    • Attend BOOST Rounds.
    • Conduct daily"rounds" with care management team to identify barriers, coach through solutions and follow-up as indicated.
    • Attend Long Length of Stay Rounds (LLOS).
    • Escalate issues to Care Management Leadership as appropriate.
    • Address customer relations issues and concerns in real-time.
    • Ensure staff completes peer review process and feedback shared with leadership.
    • Promote AtlantiCare First (services)
    • Supports all value based programs.
    • Supports and participates in process improvement initiatives.
    • Motivates, advises, and coordinates, the work of staff to promote a highly efficient and effective team.
    • Facilitates team building, professionalism, and accountability among the team
    • Possesses well developed communication, interpersonal and organizational skills.
    • Interacts effectively with all levels of the organization.
    • Demonstrates commitment to internal and external customer service and flexibility performing required duties.
    • Possesses good critical thinking, organization and problem solving skills.
  • Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards.

QUALIFICATIONS

EDUCATION: Graduate of an accredited school of Nursing, Bachelor's degree in Nursing preferred. Graduate of an accredited Social Work Program, LSW or LCSW required.



LICENSE/CERTIFICATION: Current licensure as a Registered Nurse or Social Worker in the State of New Jersey. Case Management certification required within 1 year of hire.



EXPERIENCE: 2 -- 4years care management required. Prior clinical experience maybe substituted. Well-developed communication and facilitation and analytical skills required. Proficiency in Clinical Applications preferred at time of hire; incumbents within position will be trained appropriately and then skill will be required for this position within 30-60 days from date of hire.

WORK ENVIRONMENT

Potential for exposure to the hazards and risk of the hospital environment, including exposure to infectious disease, hazardous substances, and potential injury. Position requires considerable walking and standing and some sitting at a desk and computer.

REPORTING RELATIONSHIP

This position reports to the Corporate Director of Care Coordination.
