POSITION SUMMARY:

The Health Engagement Registered Nurse will support organizational population health initiatives. Responsibilities include but are not limited to the promotion of health maintenance and disease prevention of individuals, families and the community through vaccination clinics, biometric wellness screenings, care coordination in support of the primary care medical home model, and other duties related to health education, outreach, screening and prevention.

The Health Engagement Nurse will support outcomes measurement and the creation and delivery of evidence based practices related to health education and outreach. This position will also mentor and guide other health engagement staff as necessary to assist with population health initiatives.

The Health Engagement Nurse will support organizational goals by providing quality customer service, participating in performance improvement efforts and by demonstrating a commitment to teamwork and cooperation.

QUALIFICATIONS

EDUCATION: Graduate of accredited school of nursing required. Bachelor's degree preferred

LICENSE/CERTIFICATION: Current license as a Registered Nurse in the State of New Jersey. A valid NJ driver's license required. BLSHCP required.

EXPERIENCE: Minimum of two years related experience required. Care management, public health, community nursing, or wellness preferred. Proficiency in computer skills required.

PERFORMANCE EXPECTATIONS

* Possess well developed communication, interpersonal and organizational skills.

* Interacts effectively with all levels of the organization.

* Demonstrates commitment to internal and external customer service and flexibility performing required duties.

* Possesses good critical thinking and problem solving skills.

* Collaborates with members of the Health Engagement team in making referrals to related programs and services.

* Is cost conscious and functions in a fiscally prudent way when utilizing resources.

* Performs job duties in a manner consistent with the organization's regulatory requirements and departmental standards.

* Thorough knowledge of evidence based guidelines and education related to chronic disease management

* Self-starter with the ability to work independently with little day-to-day supervision.

* Demonstrates the competencies as established on the Assessment and Evaluation Tool for this position.

WORK ENVIRONMENT

Potential for exposure to the hazards and risk of the hospital environment, including exposure to infectious disease, hazardous substances, and potential injury. This position requires sitting, standing, walking, stooping and crouching a majority of the workday. Works with such equipment as computer terminal, fax machine, printer and copier.

This position requires travel mostly in and around the AtlantiCare primary service areas as well as some travel within the regional service area of AtlantiCare. Position requires flexibility of work hours including coverage for off shift and weekend events. This position requires occasional walking and standing. May have prolonged periods of sitting and frequently works with such equipment as computer terminal/fax machine/copier/telephone.
